So for "Relationship with next of kin" (e.g. in an application form), is it their relationship to me or my relationship to them?

Update:

So for example if I put my dad as next of kin, would "Relationship with next of kin" be daughter or father?

    Their relationship to you. So you'd put "parent" or "spouse" or "cousin" or whatever.
