Some companies sell them. Email providers like Yahoo are open to hacking by computer whizzes. Sometimes malicious employees at places like Walmart sell entire lists of identification and credit cards.

Some are generated at random. However the most common source is from viral emails. You get an email with a joke or funny video with the recommendation you send it to all your contacts. It doesn't take long before these build up a huge list of email addresses that get sold on to the scammers.
