The highest percentage in terms of what? Deaths? Cases? Deaths per population? Per Capita? A Million deaths in Italy is a far higher percentage of the country's population than a million deaths in the US, which is several magnitudes of both people and land higher.

But as another poster has said, all of the data needs to be taken in context as there are different reporting methods in different places, but you can get a rough idea.

That is not possible to determine since deaths are recorded differently. Some countries do not even count anyone who was in assisted living while the US includes everyone who died of anything in assisted living as being from the virus. Some countries are hiding deaths.

If you are interested, do the math yourself.

Belgium has the highest, in Europe.

Indonesia has about reported about 13,000 cases and 1,000 deaths. That's about as close as the numbers get.
