Write the unbalanced chemical equation to represent each of the following reaction statement. Do not include phase abbreviations.
(a) Potassium hydroxide and phosphoric acid react to form potassium phosphate and water.
(b) Zinc and silver chloride react to form zinc chloride and silver.
(c) Sodium hydrogen carbonate reacts to form sodium carbonate, water, and carbon dioxide.
(d) Ammonium nitrite reacts to form nitrogen and water.
(e) Carbon dioxide and potassium hydroxide react to form potassium carbonate and water.

As a chemistry teacher for over 40 years, I would take issue with some of the instructions. You should always balance a chemical equation. You should include the state symbols. Is your teacher new to teaching chemistry? There are several more mistakes.
You are given the reactant and product names. Do you know how to right the formula from the name? You should by now.
By the way, the first one has a problem. The phosphate ion will only be found in a solution which is very basic. Instead KH2PO4 will be a more likely product, unless there is a major excess of KOH.
a. KOH(aq) + H3PO4(aq) --> KH2PO4(aq) + HOH(l)
b. This isn't likely to give a reaction since silver chloride is insoluble in water.
c. 2KHCO3(s) --> K2CO3(s) + CO2(g) + H2O(g)
d. NH4NO2(s) --> N2(g) + 2H2O(g)
e. CO2(g) + KOH(aq) --> KHCO3(aq) .......... No K2CO3, KHCO3 instead, yet another mistake
