Fuel filters and bad fuel pumps can cause a fuel starvation problem. Replace the fuel filter, and have fuel pressure checked at fuel rail to ensure the proper psi. If there is a misfire, it will cause the car to hesitate, shake, and idle very rough pretty much all the time, and it will smooth out a bit when you reach a higher speed. Replace spark plugs and wires, sometimes a bad coil is to blame, and will need replacement.
